PHN After Hours Primary Health Care SC Area

⁠⁠⁠Central Queensland, Wide Bay, Sunshine Coast PHN (the PHN) is a commissioning organisation and is inviting Proposals for initiatives to increase access, efficiency and / or effectiveness of After Hours Primary Health Care.The PHN is one of 31 Commonwealth funded PHNs nationwide. It is a not for profit organisation which coordinates primary and preventative health care for a region of 161,000km2, with a population of 823,985 people; stretching from Glasshouse Mountains in the south, to north of Yeppoon and west of Emerald.

This opportunity aims to increase the access, efficiency and effectiveness of After Hours Primary Health Care for the communities in the Sunshine Coast sub-region of the PHN. Specifically (but not limited to) those with poor access to Health Services, families with young children (0-4yo), young adults, over 65's and people with mental health issues; and improve access to After Hours Primary Health Care through effective planning, coordination and support for those living with complex and chronic conditions.

The PHN is seeking Proposals for the development and implementation of innovative After Hours Primary Health Care initiatives, that address gaps in After Hours service arrangements (particularly for priority groups) and improve service integration and continuity of care, for patients with complex and chronic conditions in the Sunshine Coast sub-region of the PHN.

Eligibility: Proposals are sought from primary care agencies which can include allied health services, community health and community pharmacy, medical support such as imaging / pathology and general practice but does not include in-hospital in-patient services.

Who should complete the application: The application must be completed by an authorised representative of the organisation. A separate submission needs to be completed for each proposal. If partnering with another organisation, the lead organisation should complete the submission.

Response requirements: Responses can be saved as you go. You can enter and exit the response as many times as you wish. Responses can be edited at any time prior to completion and submission.

Proposed Term: The proposed term of the contract arising from this Proposal is from 13/03/2017 to 30/07/2017.

Deadline for submissions: at 17:00 (5:00 PM) QLD TIME on 17 February 2017 all submissions must be fully received by this time.
