Integrated Models of Care for Older People in Residential Care

⁠⁠⁠Western Sydney Primary Health Network is seeking proposals from service providers to establish, expand and/or improve existing services to better manage non-life threatening acute conditions to residents in Residential Aged Care Facilities (RACF) within Western Sydney.

The project will work collaboratively with Western Sydney Local Health District (WSLHD) Geriatric Outreach Services, RACF management and staff, consumers and their families to deliver services which can address unnecessary hospitalisation and ambulance attendances.

Since 2002 WentWest has been part of the western Sydney community, delivering support and education to primary care and working with key partners to progress the region's health system.

As the Western Sydney Primary Health Network, WentWest is focused on addressing both regional and national health challenges. Together with health professionals, partners from both the health and hospital sector, consumers and the broader community, WentWest seeks to identify gaps and commission solutions for better health outcomes.
