Request for proposal – Pharmacy-led ACOP Integration and Support Services

⁠⁠⁠Overview

Central and Eastern Sydney PHN (CESPHN) is requesting proposals from suitably qualified pharmacy providers to deliver Pharmacy-Led ACOP Integration and Support Services across Residential Aged Care Homes (RACHs) within the CESPHN region. This 12-month initiative aims to strengthen medication management, support the effective integration of Aged Care On-site Pharmacists (ACOPs) into multidisciplinary care teams, and build workforce capability through structured mentoring, education, 

Objectives

• Support the effective placement, integration and retention of ACOP pharmacists within Residential Aged Care Homes (RACHs)
• Deliver structured workforce development to improve ACOP capability, consistency and quality of ACOP services through structured mentoring, education, and shared resources
• Facilitate collaborative practice and peer learning environments through a Community of Practice model

Scope

The scope includes:

• Delivery of Pharmacy-Led ACOP Integration and Support Services across Residential Aged Care Homes (RACHs) within the CESPHN region
• Coordination of program delivery across the region
• Provision of structured mentoring, education, and collaborative learning activities to support ACOP integration and workforce capability

Contractor Requirements

• Demonstrated experience in aged care pharmacy services
• Capacity to coordinate program delivery across the region
