Gippsland PHN Alcohol and Other Drugs: Family and Carer Support Service.

Would you like to deliver services to family, friends and carers of people misusing substances.
Do you have an innovative proposal that would benefit the Gippsland community.

Gippsland PHN is a not for profit primary health organisation with the key objectives of:
* increasing the efficiency and effectiveness of medical services for patients, particularly those at risk of poor health outcomes; and
* improving coordination of care to ensure patients receive the right care in the right place at the right time.

Proposals are now being invited for those wishing to deliver support services to family, friends and carers of people misusing substances. This proposal opportunity is open to all providers.

Funding is now available for program proposals that have a focus on supporting family, friends and carers through a multi-faceted model of individual, group and peer support. Funded programs will deliver an evidence based service. Five local government areas have been identified for place based services, with the following per annum funding available:
* South Gippsland $100,000.00
* Wellington $100,000.00
* East Gippsland $100,000.00
* Baw Baw $100,000.00
* Latrobe $100,000.00

Programs delivered through this funding will be initially contracted to 30 June 2018. Programs must be able to demonstrate evidence based outcomes, and strong linkages with the existing alcohol and other drug, health and complementing sectors, including employment and education.

Proposals are to be submitted via Tenderlink prior to 5pm on Wednesday 23rd November 2016.
