Delivery of Integrated Team Care Health Services for Aboriginal and Torres Strait Islander Peoples in Tasmania
Primary Health Tasmania is seeking tender applications from suitably qualified and experienced service providers to deliver integrated team care health services for Aboriginal and Torres Strait Islander peoples in Tasmania. The services will aim to improve health outcomes for Aboriginal and Torres Strait Islander peoples living with chronic health conditions. Applications can be from individual providers or organisations, or joint applications (e.g. from a coalition, collaboration or consortium).
Successful applicants will be funded from 1 January 2017 and will be offered a service agreement through to 30 June 2018. Key performance indicators will be used to measure services and outcomes.
