Seeking proposals to undertake an After Hours focused Comprehensive Needs Assessment and Service Mapping in the South Eastern Melbourne Primary Health Network (SEMPHN) catchment.
Proposals are sought from suitably qualified consultants to undertake a needs assessment and consequent strategic plan, which will support the implementation of integrated models aimed at building capacity to better respond to the primary health needs of people within the SEMPHN region impacting the After Hours period (AH) - The After Hours period has traditionally been defined as: before 8am and after 6pm on weekdays, before 8am and after 12pm Saturday, and all day Sunday and public holidays.
This work aims to increase the understanding of the health and social factors that influence individual access to health services across the acute and primary system. It aims to develop a deep understanding of the impact on AH health care services by:
I. Undertaking a detailed needs analysis and service mapping
II. Developing strategies to support capacity building in the primary health care and related non-health sectors to address the needs of people who are or at risk, particularly in geographical hot spots; and
III. Develop evidence based strategies to improve access to primary health care services in order to achieve optimum health management and positively influence health outcomes.
