COORDINARE South Eastern NSW PHN is seeking proposals that will promote healthy and active ageing by supporting older people to stay well and in their communities by improving:
(a) approaches to regular malnutrition screening, and/or appropriate service/system responses, in a range of settings, and/or
(b) improving access to, or availability of, evidence based exercise programs aimed at falls prevention and targeting multiple attributes including balance and strength training.
Funding is available for 18 months. Proposals must demonstrate how they will target those most at risk of poor health outcomes, outline an approach to evaluation and demonstrate alignment and integration with the broader service system.
Proposals must be lodged electronically via the portal.
Submissions must be received by 5.00 pm Wednesday 12 October 2016. Late submissions will not be accepted.
