COORDINARE South Eastern NSW PHN is approaching the market to identify initiatives with the potential to contribute to a reduction in overweight and obesity in the local population, and particularly in priority populations at greater risk of weight related chronic disease.
Funding is available for 18 months.
Proposals must demonstrate how they will target those most at risk of poor health outcomes, outline an approach to evaluation and demonstrate alignment and integration with the broader service system.
Proposals must be lodged electronically via the portal.
Submissions must be received by 5.00 pm Wednesday 12 October 2016. Late submissions will not be accepted.
