Do you have an idea to improve access to after hours primary health care in the Tasmanian community?
Primary Health Tasmania is seeking expressions of interest from individual providers and/or organisations who may wish to:
A. Work with Primary Health Tasmania in developing an innovative model of care, or
B. Deliver an innovative model of care in the community.
After Hours Service Improvement Scheme grants will be released through a request for tender process in July 2016. The current expression of interest process will help Primary Health Tasmania assess the readiness of the market to provide after hours services during the 2016-17 financial year and engage with service providers who plan to apply for a grant.
The aim of the After Hours Service Improvement Scheme is to improve access to primary health after hours care for Tasmanians, with a particular emphasis on vulnerable people. This includes the elderly, people with complex health conditions, people on low incomes, palliative care and house-bound people There is also an emphasis on areas with little or no after hours general practice coverage.
