Murrumbidgee Primary Health Network is seeking Proposals for a Frailty Intervention Service.
The aim is to halt or reverse the progression of frailty in the elderly populations of Deniliquin, Corowa, Griffith, Leeton, Narrandera, Tumut, and Cootamundra.
It is expected that the service will be delivered to 500 participants between March 2016 and June 2018. The Frailty Intervention Service will combine elements of the following:
* Exercise coaching
* Nutritional advice
* Health coaching/facilitating participation in group cultural/hobby activities and/or cognitive training
