WNSWLHD are seeking to partner with external respondents to provide Allied Health Services to the Transitional Aged Care Program (TACP) within the district.

To deliver high-quality, evidence-based, goal-oriented Allied Health based therapeutic care that supports individuals to maintain or improve functional capacity and cognitive functioning following a hospital discharge, enabling safe transition to their preferred long-term living arrangement. WNSWLHD are seeking to partner with external respondents to provide Allied Health Services to the Transitional Aged Care Program (TACP) within the WNSWLHD. The successful applicants will be registered as an associated provider for WNSWLHD TACP under the Aged Care Act 2024.

The TACP Service Provider will optimise independence and wellbeing while ensuring a multidisciplinary and therapy focussed approach to deliver seamless care.
The provision of Allied Health Services including:
- Physiotherapy
- Occupational Therapy
- Speech Pathology
- Dietetic
- Podiatry
- Social Work
- Psychology / Counselling
