Health Connection and Chronic Conditions Management (HCCCM) Services RFT

Murray PHN is looking to engage multiple providers to deliver the below services across identified areas of our region.

We invite providers to submit a Tender outlining their interest and capacity to:

• Provide Regional Health Navigation services across each defined Murray PHN geographical sub-region,

and/or

• Deliver direct clinical services through individual face to face, telehealth and/or group programs to support consumers in prioritised local government areas, who live with COPD, cardiovascular disease (focus on chronic heart failure) and/or diabetes, also ensuring management of related comorbidities and or management of risk factors to prevent the development of comorbidities.

Providers are encouraged to ensure they address all points listed in the evaluation criteria and will be required to complete Part C - Provider Information and Response Form and Attachment 1 – Proposed Service Type, Delivery Area and Targets and Attachment 2 – Budget Template as part of their submission.
