Tender Description
The Eastern Health Transition Care Program (TCP) operates under the governance of the Care @ Home and Acute Specialist Clinics program. TCP is jointly funded by the Victorian Department of Health and the Australian Government Department of Health, Disability and Ageing.
TCP provides short-term care designed to optimise the functioning and independence of older people following a hospital stay. The program is person-centred, goal-oriented, time-limited, and therapy-focused, offering a package of services that may include:
- Low-intensity therapy (physiotherapy, occupational therapy, speech pathology, dietetics)
- Allied health assistant support
- Case management
- Nursing care
- Personal care
The primary aim of TCP is to prevent individuals from prematurely entering residential aged care after hospitalisation and to support transition, either back home or into long-term care.
The length of stay in TCP should not exceed 12 weeks, unless an extension of up to 6 weeks is approved by the Aged Care Assessment Service delegate.
Applications are invited from one or more providers to become an associate provider for Eastern Health TCP to supply a minimum of 15 aged care beds at a single facility, up to a total of 65 beds for a 3 year Contract term with 1 x 2 year extension option. To ensure the highest quality of care, beds must be located in a single area within the facility. Suppliers must be a registered residential aged care provider capable of delivering TCP services compliant with the Aged Care Act 2024, Aged Care Rules 2025 and the Strengthened Aged Care Quality Standards.

Included in the attachments is an instruction guide to complete this process. This advance tender notice is issued for market awareness only. It is not the tender, and no tender documentation is being released at this stage.
Tender Timeline
It is expected that the Tender will be released during the last week of January 2026 with the awarded contract/s commencement of services in August 2026.
