Request for Tender - MDT Melton Neighbourhood Care Team

NWMPHN seeks to commission one suitable organisation to provide a highly mobile two-person team to work with selected general practices within the Melton Local Government Area (LGA). This team, known as the Neighbourhood Care Team (NCT) will play a key role in delivering multidisciplinary care to patients with complex needs.

The NCT will comprise of a clinical care coordinator and a social worker. They will meet every week with clinicians and patients at three Melton practices. They will assist staff in assessing patient need, delivering care, supporting patient self-management, and coordinating access to other allied health and support services in the local area.

Eligibility

Hospitals and acute care service providers are not eligible to apply for this funding. This RFT is intended for primary care providers only. However, if an organisation classified as a hospital or acute care provider also operates community health or primary care services within Melton, it may be eligible but only if it can clearly demonstrate that those services meet all required capabilities outlined in this RFT.

Additionally, providers currently applying to participate in the project as a general practice via Request for Information (upcoming tender) are not eligible to apply for this funding.
