Health Connection & Chronic Care Management Request for Tender Information Briefing

Murray PHN will be releasing a request for tender for Health Connection & Chronic Care Management Services in late January 2026.

The Health Connection & Chronic Care Management Services program will prioritise chronic diseases of highest regional impact such as COPD, diabetes, and cardiovascular disease.

The Program will focus on place-based delivery and directed towards priority locations and service types based on multiple sources of data, including; rates of chronic disease, socioeconomic disadvantage, low English proficiency, potentially preventable hospitalisations for chronic conditions, existing health infrastructure, service availability/distance to services and service gaps.

There will be an online Briefing Session via Teams on the 22nd of January 2026 @12:00pm before the release of the tender documents.

The tender will be released on the 23rd of January 2026 and will be open for four weeks closing on 20th February 2026. It is anticipated that outcomes from this RFT process will be Communicated in Late March 2026.
