Northern Health advises that the Invitation to Supply (ITS) for the Provision of Transition Care Program and Geriatric Evaluation and Management at Residential Care Services has now been formally released to the market.

Northern Health is seeking submissions from suitably qualified and accredited Residential Aged Care Providers to deliver short-term transitional and restorative care for older persons following an acute or sub-acute hospital episode. The successful provider(s) will be required to supply up to twenty-four (24) dedicated residential care beds, with the capacity to accommodate dementia-specific pathways and provide scalable surge capacity over the contract term.

The appointed provider(s) will be responsible for delivering safe, high-quality residential aged care services including:

• 24/7 nursing and personal care

• Cleaning, laundry, catering and environmental services

• Consumables, equipment, linen and facility amenities

• Appropriate therapeutic, communal and administrative spaces

• Collaboration with Northern Health Geriatricians, Care Coordinators and Allied Health teams
