Unified Program Services Request for Expressions of Interest

Eastern Melbourne Primary Health Network (EMPHN) invites expressions of interest responses for the provision of Unified Program Services, in accordance with the requirements of this Request for Expressions of Interest (REOI).

Throughout 2025, EMPHN has worked with providers, consumers and stakeholders to design a commissioning model that enables place-based, integrated care and fosters innovation. The Unified Program is a strategic shift in how primary health care is delivered across our catchment.

EMPHN invites expressions of interest from service providers—whether solo organisations or collaborative partnerships—who are ready to deliver now and evolve with us as the model expands.

This is an opportunity to help co-create a future-ready system that is:

• Equity-driven – centring care around hardly-reached communities, including First Nations peoples, CALD and LGBTQIA+ populations, and those facing systemic disadvantage
• Integrated and place-based – delivering multidisciplinary care that reflects local needs and connects seamlessly with general practice and the broader health system
• Flexible and outcomes-focused – adapting to community strengths while being accountable for meaningful health outcomes
• Collaborative – forging strong partnerships across hospitals, general practice, equity and specialist providers, and community organisations
• Innovative – co-designing with consumers and using outcome-linked funding to test and scale new approaches
• System-oriented – strengthening governance, networks, and accountability to drive collective impact

Responses are required to be lodged via the online form, by the advertised closing date and time. Please ensure you read all parts of the REOI to understand the process for lodging a response.

EMPHN will hold an industry briefing session following the release of this REOI.
