Tender Description
Gippsland PHN is seeking to commission a mobile outreach healthcare service that delivers culturally responsive, trauma-informed care to people experiencing or at risk of homelessness in the Wellington and East Gippsland Local Government Areas.
The service will operate in community settings and provide primary healthcare and facilitated connections to social and housing services. The successful provider will be expected to deliver a multidisciplinary, evidence-based model that is free for clients and aligns with the Quintuple Aim of Healthcare.
