Gippsland PHN are seeking to commission a lead community health service or health service provider consortium to develop and implement an integrated place-based outreach model of care in East Gippsland to support Victim-Survivors of Family, Domestic and Sexual Violence, in particular those from population groups at disproportionately higher risk of Family, Domestic and Sexual Violence. This will include the formation of a multi-disciplinary care team that includes:

- Intake services for the consumer and their people,

- Care connectors and/or care coordinators to provide care coordination services and access to primary health care,

- Nurse Practitioners, Registered Nurses, Mental Health Nurses,

- General Practitioners,

- Mental Health Clinicians,

- Allied Health,

- Maternal health, and

- Paediatric and associated and early childhood services.

The composition of the team will be reflective of current identified needs of Family, Domestic and Sexual Violence victim-survivors in Gippsland. The team will have understanding and experience of, or be trained in, trauma informed and responsive care.

Services are expected to be delivered from 11 August 2025 to 30 June 2027.
