Tender Description

⁠⁠⁠Department for Health and Wellbeing are seeking to contract a panel of service providers to deliver the Discharge to Respite Program, which will streamline the pathway for older adults in public hospital who are either on a discharge pathway to permanent residential aged care placement or require a period of respite prior to returning home.
