Grampians Health is seeking a suitably qualified and experienced service provider or group of service providers to supply the organization with the expertise to conduct Residential Medication Management Review Services and Associated Quality Use of Medicine Services.
This tender shall cover the services to suit the processes undertaken in reviewing client's medication within the Residential Aged Care Facilities of Grampians Health - Ballarat, Edenhhope and Stawell - a total of 12 facilities.
