EXPRESSION OF INTEREST - CONSULTANT - COMMUNITY-BASED MODELS OF HEALTHCARE FOR EARLY DETECTION OF CHRONIC DISEASE IN LATROBE.

Latrobe Health Assembly (LHA) is seeking to engage a consultant to undertake research into existing and previous community-based models of healthcare for early detection of chronic disease (e.g. mobile clinic or service hub) across local and international services.

The Latrobe Valley has been designated as a Health Innovation Zone (the Zone), the first of its kind in Australia. The Zone was established by the Victorian Government to drive innovation and change leading to improved health and wellbeing outcomes for residents in Latrobe City.

It is expected that the consultant will work closely with partner organisations to look into existing and previous community-based models of healthcare for early detection of chronic disease (eg. mobile clinic or service hub) including published literature, grey literature and case studies.

The consultant would also be responsible for utilising the learnings and recommendations to develop a concept to be piloted through a co-design process.
