Tender Description
The PHN seeks experienced organisations with capability and capacity to deliver the Clinical Care Coordination service.
As part of the continued reform of Mental Health Services, the PHN will commission Clinical Care Coordination. It is anticipated that this service will complement existing mental health programs and expand the current Clinical Care Coordination that are delivered in Primary Care to include a broader multidisciplinary workforce. Services are to be provided to vulnerable groups living in the Hunter (not including Mid-coast), New England, and Central Coast regions.
