Tender Description

⁠⁠⁠The Flexible Home Support Services (FHSS) Program  provides support and care coordination  for older people who are transitioning to more permanent commonwealth support services. Clients will have had an assessment by staff of the WNSWLHD Aged Care Division and a summary and recommendations for care will be provided at the time of the referral to the Provider.
