Tender Description

⁠⁠⁠Primary Health Tasmania seeks to commission suitable service provider/s to deliver a care finder program.

This service will be available across Tasmania and will commence service delivery by April 2023 and will be commissioned to June 2025.

The service will:

-provide specialist and intensive assistance to help the target population understand and access aged care services and connect with other relevant supports in the community.

- specifically assist those people within the target population to access My Aged Care.
