This grant opportunity will expand on the 2020-21 Budget measure that is currently funding five well-developed proof-of-concept pilots into fully operational models customised to address the primary care issues in proposed sites in Western and Southern NSW. This grant opportunity will fund new innovative models of care in rural jurisdictions outside of NSW in Modified Monash Model (MMM) locations MM 3 – 7.
The aim of the grant opportunity is to enable the implementation of new innovative primary care service delivery models, and determine whether these models improve the attractiveness of rural practice and lead to better delivery of health services to communities in regional, rural and remote Australia (MM 3 – 7 locations).
