The Social Connection Partnership Grant (Great South Coast) will be allocated to the supplier/s who are able to expend the funds on expanding and improving existing initiatives already underway locally.
Initiatives should include some elements of direct service delivery and contribute to immediately improving social connection and reducing social isolation within the community.
The aims of the Social Connection Partnership Grant are to:
- Improve social connection in the WVPHN community through available virtual services and other initiatives which address the social isolation caused due to COVID-19.
- Foster the development of cross-sectorial collaboration and partnerships to implement locally responsive innovations that build on existing activities and address the social and economic impacts of social isolation currently emerging.
- Strengthen relationships across sectors and in the community to identify co-investment
opportunities to broaden the scope of partnerships and work towards implementing a social
prescribing service model in the future.
- Increase collaboration with mental health services (including those delivered through WVPHN) and other program areas to align similar work, maximise impacts and reduce funding duplication.
