Tender Description
Western Victoria PHN is seeking an independent evaluator to conduct an evaluation of the Community Transport Service for Rural Victoria Project at both an individual transport hub level and at an overall project level. The evaluation will be used to determine the impact on service users, community and the health care system; scalability and replicability; and the sustainability of the transport model beyond the funding period.
