Eastern Melbourne PHN (EMPHN) would like to invite responses to an Expression of Interest for the Vulnerable People’s Homebound Vaccination Program in accordance with the requirements of this Expression of Interest (EOI)

The Australian Government has provided EMPHN with funding to commission additional support for innovative, local, short-term activities. These activities will aim to provide vaccinations to vulnerable populations identified as having difficulty accessing COVID-19 vaccinations.

The vulnerable target population at focus are people who may be homebound and cannot leave home to receive a COVID 19 vaccination due to their circumstances. This includes but is not limited to the frail aged and elderly, individuals living with chronic or severe conditions, people living with a disability or a mental health condition that prevents them from leaving their home.

EMPHN is partnering with North Western Melbourne Primary Health Network (NWMPHN) and South Eastern Primary Health Network (SEMPHN) to facilitate access to the vaccination for homebound persons.
