Capacity Tracker Consultancy Support

Gippsland PHN is seeking support from a consultant to assist with the implementation of the Capacity Tracker initiative across the Gippsland region, working with General Practice, Residential Aged Care Facilities (RACF) and Community Pharmacy.

Proposals are sought from suitably experienced and qualified organisations to assist Gippsland PHN in enhancing the implementation, promoting uptake and conducting a robust evaluation of the Capacity Tracker initiative in Gippsland.

The consultancy contract will commence from May 2021 and is required to be completed by 31 December 2021.
Gippsland PHN's total budget for this procurement, including all related expenditure, is $240,000.00 (ex. GST).

Tenderers should address the evaluation criteria detailed in the Tender Specification. A completed Tender Application Form, along with any other relevant documentation, must be uploaded to Gippsland PHN's etendering portal by the closing time of 5:00pm AEST on Friday 30 April 2021.
