Mental Health In-reach Services and Support for Older Australians

⁠⁠⁠The Hunter New England Central Coast Primary Health Network (HNECCPHN or The PHN) is seeking Expressions of Interest (EOI) from Applicants to provide Mental Health Services and In-reach Support for Older Australians who are at risk of, or have, mental health issues and who are experiencing social isolation and/or loneliness as a consequent of the pandemic.

By the end of the funding period, the initiative will deliver the following desired outcomes for older Australians, as well as their carers:
- Connecting older Australians to appropriate services and support
- Reconnecting older Australians to social networks
- Restoring access to service that may have been disrupted by the pandemic
- Provide psychosocial information to friends, family and carers on the wellbeing of older Australians.
Applicants are required to complete Segment C EOI Returnable Schedules.
