Tender Description
HNECC PHN intends to invite suitably qualified and experienced Providers to tender for the delivery of Clinical Care Coordination in the Mid Coast region as part of the broader Mental Health Services in Primary Care being delivered to mainstream populations and RACFs across the HNECC region.
A physical presence established office space or co location in the Mid Coast region is required with capability to deliver the Services from 1st April 2020.
The successful Provider will be provided a 15 month contract with service delivery commencing in an establishment phase from 1st April 2021. The contract end date is 30 June 2022.
