Barwon Health is seeking responses to this Request for Tender (RFT) BH_20120-1017 for the supply of at home community support services. The Barwon Health programs that will engage in the brokerage of these services are the Post-Acute Care services, Transition Care Programs, Carer Support and Hospital in the Home (HITH).
Barwon Health’s Hospital in the Home Program provides hospital health care in the home as an alternative to being treated in hospital. The aim of the program is to enable patients who are medically stable to be treated at home or transferred home from hospital earlier than has previously been possible and link these patients into a range of services according to need.
