Person-centred care Health Literacy Activity

⁠⁠⁠Primary Health Networks (PHNs) have been established with the key objective of increasing the efficiency and effectiveness of health services for patients, particularly those at risk of poor health outcomes, and improving coordination of care to ensure clients receive the right care in the right place at the right time.

The Person-Centred Care Initiative equates to a substantial and purposeful investment in transforming and supporting higher performing primary care in the Brisbane south region. Improving health literacy and self-management of consumers is a core element of the Person-Centred Care Model and is a key organisational focus. The development of a strategy using human-centric design principles and the conducting of direct consumer engagement is beyond the capability and capacity of the Brisbane South PHN workforce and therefore Brisbane South PHN seeks suitably qualified applicants to respond to provide these Services.
