Panel of Approved Providers - Provision of Commonwealth Home Support Program In-home Service

⁠⁠⁠City of Ballarat is seeking submissions from suitably qualified and experienced individuals and companies for the provision of all in-home services, these being but not exclusive to; domestic assistance, personal care, shopping both from list and assisted, medication prompts, transportation to medical and other appointments, day respite arrangements to eligible and approved people over the age of 65 years, under the Commonwealth Home Support Program (CHSP), residing in the City of Ballarat, including its townships and villages.
