Provision Of Community Support For Medically Complex Children
Tender ID: 405676
Tender Details
Tender Description
The Child and Adolescent Health Service (CAHS) has identified a continual need for the provision of support for medically complex children in the community setting.
The proposed services are specifically for medically-stable children who no longer require tertiary-level care at Perth Children’s Hospital (PCH) but require planned transition to a home or residential care setting. Based on individual circumstances, residential and in-home care requirements will be as follows:
Residential Care will be required for respite and/or medium-to-long-term placement options in a place designed for children to live and have care provided on-site (24-hours a day, 7-days a week), and provides training for families and carers to support the transition home;
In-Home Care includes support from trained and competent support workers. This type of care usually involves a support worker to provide awake overnight supervision. The child may also need daytime care in the home, school or other community activities. The hours of care required will be assessed on a case by case basis and will vary according to the children’s need.
The service provisions are required to comply with Quality Standards and Operational Requirement, which will be detailed in the Specifications.
The choice of supplier(s) will be based on specifications for the individual child at the point of care. The level of care provision required will be based on an assessment of the family’s individual needs.
