Tender Description
To identify a competent Service Provider to design and implement a pilot Youth Worker Model that provides long term intensive case coordination and navigation support for young people leaving Juvenile Justice.
The model shall include but not limited to the following:
- Trauma informed care and practice
- Transition planning
- Wrap around support
- Youth centred and family focused
- Individualised and tailored service delivery
- Specialised Aboriginal and Torres Strait Youth worker
- Clinical oversight
- Highly skilled and qualified Youth Workers
WSPHN (WentWest) is seeking innovative and scalable proposals that addresses Alcohol and Other Drugs (AOD) and barriers faced by the young people leaving the Juvenile Justice system in the western Sydney region.
The model to incorporate a cultural model addressing the needs for young Aboriginal and Torres Strait Islander leaving the Justice system.
