Tender Description
Primary Health Networks (PHNs) were established with the key objectives of increasing the efficiency and effectiveness of medical services for residents, particularly those at risk of poor health outcomes, and improving coordination of care to ensure residents receive the right care in the right place at the right time.
The South Eastern Melbourne PHN catchment encompasses an area comprising 1.5 million people and stretches from St Kilda to Sorrento and as far east as Bunyip, including the major population hubs of Monash, Dandenong, Moorabbin, Caulfield, Cranbourne, Frankston and Pakenham.
From 1 July, 2016 PHNs adopted a commissioning approach to procuring medical and health care services which is based on an assessment of the health and wellbeing needs of the community. This work determines the priority areas and guides the scope of the commissioning process.
The South Eastern Melbourne Primary Health Network (SEMPHN) is pleased to invite requests to deliver the Residential Aged Care Facilities Psychological Services (the ‘Service’).
The Commonwealth Government has provided funding through regionally-based Primary Health Networks to commission new mental health services for older people with low to mild mental illness who reside in Residential Aged Care Facilities. This will help to address a current gap in services for this group of people and offer access to psychological services which are similar to those available to people with mental illness in the community under Medicare arrangements.
SEMPHN is seeking to engage an organisation to deliver Residential Aged Care Facilities Psychological Interventions services across all South Eastern Melbourne Local Government Areas, excluding Mornington Peninsula. South East Melbourne LGA’s include: Bayside, Glen Eira, Stonnington, Port Phillip, Dandenong, Casey, Cardinia, Kingston, and Frankston.
Funding applications from consortiums and partnerships will not be considered.
The successful Service Provider will be contracted from June 2019. The service will be implemented in a staged approach with the services commencing July 2019.
