Can your organisation work closely with Indigenous health and community services and local communities to deliver quality outcomes?
Gippsland PHN is a not for profit primary health organisation with the key objectives of:
* Increasing the efficiency and effectiveness of medical services for patients, particularly those at risk of poor health outcomes; and
* Improving coordination of care to ensure patients receive the right care in the right place at the right time.
Proposals are sought from suitably qualified providers of clinical dual diagnosis services to deliver a clinical service for Aboriginal and Torres Strait Islander people in East Gippsland, Wellington and Latrobe Local Government Areas.
Total indicative funding for this program is $292,330.99 (ex GST) per annum.
Applications are open from Tuesday 3 July 2018, closing strictly 5.00pm (AET) on Friday 20 July 2018.
