Monash Health are seeking suitable suppliers to provide brokered services in Speech Pathology to patients.
The scope of the project includes providing services to the following Monash Health departments and community-based programs:
* Post-Acute Care (PAC)
* Sub Post-Acute Care (SPAC)
* Community Support Options (CSO)
* Hospital in the Home (HITH)
* Residential InReach (RESI)
* Rehabilitation in the Home (RITH)
* Disability Services
* Complex Care
* Residential Services (including all Monash Health residential aged care facilities)
* Transition Care Program (TCP)
* In-Patient Services (including Acute, Sub Acute, Emergency and Mental Health), in situations where additional resources are required
* Other community-based programs (including those funded under Home and Community Care and the Commonwealth Home Support Program)
