After Hours GP Led Telephone Triage and Attendance Service for Residential Aged Care Facilities

⁠⁠⁠South Eastern Melbourne Primary Health Network (SEMPHN) invites Approved Medical Deputising Services (AMDS - Can be a General Practice with appropriate AMDS Licence) to apply for grants for the After Hours GP Led Telephone Triage and attendance service to Residential Aged Care Facilities (RACF).

SEMPHN is offering grants of up to $150,000 (GST exclusive) per successful eligible AMDS to provide services to RACFs in the Local Government Areas of Bayside, Kingston, Greater Dandenong, Frankston, Casey and Mornington Peninsula over an 18 month period, that include the following key services:
* A first point of call telephone Triage service delivered by a Vocationally Registered general practitioner; and,
* GP Attendance service by VR GPs or Non VR GPs (if Non VR GP attending, remote supervision form VR GP required) to Residential Aged Care facilities as required in the After Horus period.

It will be at SEMPHN's discretion to award one, multiple or no grants for this tender.

Tenders close at 3.00pm AEST on Monday 7 May 2018 and submissions must be fully received by this time. Completing an application provides no guarantee of being granted funding. Late, verbal or emailed applications will not be accepted.
