Primary Health Networks have been tasked to implement innovative and locally tailored solutions for after-hours services based on community need and to ensure patients receive the right care, delivered by the right provider, in the right place and at the right time. EMPHN after-hours funding is used to address gaps in after-hours service provision, targeting solutions for ‘at risk’ populations, and improving service integration, particularly where gaps exist due to a lack of access to general practices registered for the After Hours Practice Incentive Programme (PIP).
After hours primary care has the potential to improve consumer access to services and reduce the burden on hospital emergency departments.
EMPHN is seeking responses from suitably qualified suppliers for the provision of Innovative after-hours healthcare solutions for geographically isolated communities in the outer east and outer north and vulnerable populations including the homeless, palliative, CALD/Refugee, older population, residents in RACFs, people with disabilities, 0-4 population in the Northern Area and those experiencing mental health and AOD issues, including young people.
