The increasing prevalence of chronic disease amongst an ageing population is a worldwide challenge. Remote Patient Monitoring provides one component of a Barwon Health framework that is aimed at supporting people within the community, wherever possible, and preventing hospital admission.
With today’s technology, advancing models of care such as remote patient monitoring can now can be used to enhance the client’s ability to be monitored in the home, increasing each client’s self-management of their own care. This will also enhance patient centred care, and help clients avoid hospitalisation. Barwon Health patients living in rural and remote areas will also be able to get the care they need, without traveling to a Barwon Health facility.
