MATRIX Model: Intensive Outpatient AOD Treatment for Adults - Frankston

⁠⁠⁠The South Eastern Melbourne Primary Health Network (SEMPHN) is pleased to invite Expressions of Interest (EOI) from organisations to deliver the Matrix Intensive Outpatient (IOP) AOD Treatment Service for Adults (the 'Service') to consumers within the SEMPHN catchment.

SEMPHN is seeking to engage one or more organisations to deliver the Service in the Local Government Areas (LGAs) of Port Phillip, Dandenong, and Frankston within our catchment. Funding of up to $455,000 per LGA will be provided to deliver the Service.

Following the completion of the EOI process, SEMPHN may opt to enter a closed or select tender approach for the procurement of the Service.
The Successful Service Provider will be contracted from 01 May 2018. Following a service establishment period, the Services will be delivered from 01 August 2018 through to 31 December 2019 (subject to confirmation of funding from the Commonwealth Department of Health).

NOTE: A mandatory Information Session will be held on 5 February 2018.

Organisations submitting an EOI who fail to attend the mandatory information session will not be considered.
