Western Sydney Primary Health Network (WSPHN) are looking for services that integrate care and support across co-occurring with severe and complex mental health conditions and alcohol and other drug conditions so that service users are at the centre of the model of care, getting the right care, at the right time, in the right environment, appropriate to their own individual needs. This may be a single wrap around service or care coordination within a consortium approach, for example. Residential rehabilitation services are excluded from this Request for Proposal.
Since 2002 WentWest has been part of the western Sydney community, delivering support and education to primary care and working with key partners to progress the region's health system.
As the Western Sydney Primary Health Network, WentWest is focused on addressing both regional and national health challenges. Together with health professionals, partners from both t he health and hospital sector, consumers and the broader community, WentWest seeks to identify gaps and commission solutions for better health outcomes.
Submissions close at 4:00 PM AEDT on Friday 22nd December 2017 and submissions must be fully received by this time.
