SWSPHN is offering grants to interested parties in the Bankstown, Fairfield, Liverpool, Campbelltown, Camden and Wollondilly Local Government Areas (LGA) to increase effective, affordable and accessible primary care services in the after hours period. Grant funding is available to support the establishment of practice or telephone based after hours cooperative arrangements between existing solo and small practices that otherwise have limited capacity to extend their operating hours. For the purpose of this EOI a small practice is defined as employing 9 or fewer full time equivalent GPs.
Cooperative arrangements are defined as general practices working together to provide care to patients outside the normal opening hours of their practices.
The key objectives the PHN is seeking to address through commissioning After Hours Primary Health Care services are to:
* Increase the efficiency and effectiveness of After Hours Primary Health Care for patients, particularly those with limited access to Health Services
* Improve access to After Hours Primary Health Care through effective planning, coordination and support for population based After Hours Primary Health Care.
