Tender Description
South Eastern Melbourne Primary Health Network is seeking proposals from suitably qualified consultants to undertake a Chronic Disease Service Mapping Project for South Eastern Melbourne Primary Health Network (SEMPHN) for a grant of between $300,000 and $500,000. This work will contribute to SEMPHN improving Chronic Disease services for people within the region.

South Eastern Melbourne Primary Health Network is not bound to accept the lowest priced Tender or all or any part of a Tender and reserves the right to conduct the Request for Tender process and select the successful respondent(s) as it sees fit. South Eastern Melbourne Primary Health Network staff are unable to respond to verbal enquiries in relation to the proposal.
