CSAPHN aims to bridge the gap of health inequity and access in rural South Australia by building a collaborative and responsive health care system and we are committed to improving the patient experience.
SAPHN is endeavouring to improve access to primary health services that are additional to General Practice through the IPHCS Tender, which has a focus on improving health outcomes of country South Australians, particularly for those with physical chronic and complex conditions.
The following health professions will be funded under the IPHCS, with service availability and level of service being reflective of the spread of population across the region:
* Specialist Nurse Roles in the field of Chronic Disease Management including (but not limited to) asthma, respiratory, diabetes educator and cardiovascular, chronic kidney disease, lifestyle coaching / support
* Dietitian and/or Nutritionist
* Podiatrist
* Physiotherapist / Exercise Physiologist
* Mental Health Clinician (nurse / counsellor / psychologist)
The Integrating Primary Health Care activity has been designed to enable delivery of services in rural and remote populations of 5,000 or less in the primary health care space to address the scarcity of services delivered that support patients to achieve the best outcomes for their health where physical chronic disease is the main issue.
