Improving Access to Primary Care Services for Older People

⁠⁠⁠Western Sydney Primary Health Network is seeking proposals to improve provision of evidence based group programs, interventions and/or therapies which address falls risk factors in people 65+ (45+ in Aboriginal Torres Strait Islanders) living in their own homes in the community.

Successful proposals from allied health service providers or suitably qualified health professionals may include establishment, expansion or improvement of existing evidence-based group programs and/or therapy services.

Since 2002 WentWest has been part of the western Sydney community, delivering support and education to primary care and working with key partners to progress the region's health system.

As the Western Sydney Primary Health Network, WentWest is focused on addressing both regional and national health challenges. Together with health professionals, partners from both the health and hospital sector, consumers and the broader community, WentWest seeks to identify gaps and commission solutions for better health outcomes.

Submissions close at 5.00 pm AEDT on 25 January 2017 and submissions must be fully received by this time.
