Provision of Equipment Loan Services to Colac Area Health

⁠⁠⁠Colac Area Health is an integrated health and community care facility at the gateway of the Otway National Park - 2 hours Southwest of Melbourne Victoria Australia. Colac Area Health is seeking expressions of interest from suitable services to provide equipment to satisfy our legislative requirement to provide equipment for safe discharge from our Acute and Urgent Care settings, equipment for our Palliative Care patients for at home support, and our aged care resident equipment requirements within the premises of Corangamarah, our co located Aged Care Facility.

About Colac Area Health

Colac Area Health has a 24 bed acute ward that is staffed by registered and enrolled nurses, and registered allied health practitioners. The Urgent Care Centre is a private centre that is staffed 24 hours a day and has 6 bays.

Corangamarah is an accredited public sector residential aged care service with 60 residential care beds including single, special needs, and respite care. It is a modern, safe and friendly environment with 24 hour care by registered and enrolled nurses and lifestyle workers.

Expressions of Interest are sought as follows:

The provision of all loan Equipment requirements to those eligible patients and residents of Colac Area Health.

Services provided will include:
* Provider of choice for loan equipment requirements for Corangamarah residents to assist their Activities of Daily Livings as prescribed by a registered Allied Health practitioner or Corangamarah Care Coordinator
* Provider of choice for loan equipment requirements for a period of 30 days post discharge from Colac Area Health Acute ward as prescribed by a Medical or Allied Health Practitioner
* Provider of choice for loan equipment requirements for support of home based end of life patient management  as prescribed by Palliative Care Nursing staff
* Provision of all loan equipment requirements as prescribed to enable timely discharge
* Recommended provider d hire equipment to patients, clients and residents if equipment is required beyond the 30 day post discharge.
* Recommended provider of hire equipment requirements post discharge from Colac Area Health Urgent Care centre as prescribed by Medical or Allied Health Practitioner.

Documentation Requirements:
* Invoice with patient details and patient type for billing purposes
* Fulfil all equipment loan requirement as prescribed to enable timely discharge as per legislative requirements

A sustainable equipment loan model to be developed that identifies the establishment and processes required to set up this service, in consultation with the Manager of Acute, Urgent Care, Palliative Care and Corangamarah

Itemising indicative rates applicable to service provision within a health service
Itemising indicative rates applicable to consumables required to provide services
